摘要

总结1例69岁长期口服激素致免疫功能低下、感染耶氏肺孢子菌后出现重症肺炎合并多器官功能不全患者的中西医结合护理经验。护理评估涵盖呼吸、循环、营养、凝血、皮肤及中医证候，中医证候由痰浊阻肺演变为痰热壅肺；护理诊断包括气体交换受损、清理呼吸道无效及营养失调等；护理计划为实施个体化无创通气、抗感染、抗凝、保肝及营养支持，并分期应用耳穴压豆、中药热奄包及辨证中药内服治疗。经27 d护理，患者呼吸功能改善，多器官功能好转后转院。提示中西医结合护理可发挥协同增效作用，适宜在ICU推广应用。

Abstract

This paper summarizes the nursing experience of integrating traditional Chinese and Western medicine in the care of a 69-year-old immunocompromised patient who developed severe pneumonia with multiple organ dysfunction after Pneumocystis infection due to long-term oral corticosteroid therapy. Nursing assessment encompassed respiratory， circulatory， nutritional， coagulation， and integumentary functions， along with Traditional Chinese Medicine （TCM） syndrome differentiation， with the TCM pattern evolving from phlegm-turbidity obstructing the lung to phlegm-heat congesting the lung. Nursing diagnoses included impaired gas exchange， ineffective airway clearance， and imbalanced nutrition. The care plan incorporated individualized non-invasive ventilation， antimicrobial therapy， anticoagulation， hepatoprotective measures， and nutritional support， complemented by stage-specific auricular acupressure， TCM herbal hot compress therapy， and syndrome-differentiated oral administration of Chinese herbal medicine. Following 27 days of nursing intervention， the patient's respiratory function improved and multi-organ function demonstrated recovery before being transferred to another hospital. This case indicates that integrated TCM-Western medicine nursing exerts a synergistic therapeutic effect and is suitable for promotion and application in the intensive care unit （ICU）.
