摘要

本文总结1例湿热下注型混合痔围手术期中西医结合护理方法与临床应用效果。选取1例混合痔合并肛乳头肥大（湿热下注证）手术患者，围手术期实施西医常规专科护理，联合中药坐浴、穴位贴敷、耳穴压丸、腹部穴位按摩及辨证膳食指导等中医特色护理措施，围绕术后疼痛管控、创面修复、排便管理与并发症预防开展整体护理。患者术后疼痛逐步缓解，肛周水肿、肛周湿疹消退，排便通畅，未发生切口大出血、创面感染、肛门狭窄等并发症，住院4 d后痊愈出院，出院时已熟练掌握居家康复护理要点。表明中西医结合护理可有效减轻混合痔术后不适症状、降低并发症发生率、加速创面愈合，临床实用性强，适宜在肛肠科临床推广应用。

Abstract

This paper summarizes the perioperative integrated traditional Chinese and Western medicine nursing methods and clinical outcomes for a patient with mixed hemorrhoids （damp-heat pouring downward syndrome）. One surgical patient with mixed hemorrhoids complicated by anal papilla hypertrophy （damp-heat pouring downward syndrome） was selected. During the perioperative period， conventional Western medicine specialist nursing was implemented， combined with characteristic traditional Chinese medicine （TCM） nursing interventions， including TCM herbal sitz baths， acupoint application， auricular acupressure， abdominal acupoint massage， and syndrome-differentiated dietary guidance. Holistic nursing focused on postoperative pain management， wound healing， bowel movement management， and complication prevention was carried out. The patient's postoperative pain was progressively relieved， perianal edema and perianal eczema resolved， bowel movements became smooth， and no complications such as massive wound bleeding， wound infection， or anal stenosis occurred. The patient was cured and discharged after 4 days of hospitalization and had proficiently mastered the key points of home-based rehabilitation nursing at discharge. This case demonstrates that integrated TCM-WM nursing can effectively alleviate postoperative discomfort， reduce the incidence of complications， and accelerate wound healing in patients with mixed hemorrhoids. It has strong clinical practicality and is suitable for promotion and application in anorectal clinical practice.
