摘要

本文总结1例下肢静脉性溃疡伴感染患者的中西医结合护理经验。在常规抗感染及改善静脉回流治疗的基础上，结合中医辨证结果，实施中药化腐清创术清除创面坏死组织，局部外用重组人碱性成纤维细胞生长因子及湿润烧伤膏促进创面愈合，辅以四妙散合五味消毒饮加减内服，联合中药塌渍、热奄包等中医外治疗法同步干预。同时全程开展创面动态评估、体位管理、情志护理及饮食调护。通过系统干预，有效控制创面感染，促进肉芽组织生长，减轻患者创面疼痛，对提升创面愈合质量起到积极作用。

Abstract

This article summarizes the nursing experience of integrated traditional Chinese and Western medicine （TCM-WM） for a patient with a lower extremity venous ulcer complicated by infection. On the basis of routine anti-infective treatment and therapies to improve venous return， nursing interventions grounded in TCM syndrome differentiation were implemented， including herbal debridement to remove necrotic tissue from the wound surface， topical application of recombinant human basic fibroblast growth factor （rh-bFGF） and Moist Exposed Burn Ointment （MEBO） to promote wound healing， oral administration of modified Simiao San combined with Wuwei Xiaodu Yin， and TCM external therapies such as herbal soaking compress and hot herbal compress packages. Throughout the course of treatment， dynamic wound assessment， postural management， emotional nursing based on TCM theory， and dietary regulation were simultaneously carried out. Through this systematic intervention， wound infection was effectively controlled， granulation tissue growth was promoted， wound pain was alleviated， and the overall quality of wound healing was significantly improved.
