摘要

目的

探讨在健脾和中理论指导下，腹部按摩联合穴位贴热敷在1例支原体肺炎患儿静脉输注阿奇霉素所致胃肠道反应中的临床应用效果。

方法

经四诊合参、脏腑辨证，为患儿实施调气—健脾—温阳三步法干预，干预措施包括腹部按摩、穴位贴敷及热水袋热敷，以达到理气通腑、健脾胃固中气与温运脾阳的作用。

结果

患儿胃肠道反应的不适症状得到有效改善，恶心、呕吐、腹痛及腹泻症状明显缓解，食欲改善，对后续治疗的依从性良好。

结论

基于健脾和中理论指导下的腹部按摩联合穴位贴热敷干预方案，可有效缓解阿奇霉素所致胃肠道反应，操作安全、简单且无创，患儿接受度高，值得临床推广应用。

Abstract

Objective

To explore the clinical efficacy of abdominal massage combined with acupoint application and hot compress guided by the theory of strengthening the spleen and harmonizing the middle jiao in managing gastrointestinal reactions induced by intravenous azithromycin in a pediatric patient with Mycoplasma pneumoniae pneumonia.

Methods

Based on the four diagnostic methods and visceral pattern differentiation， a three-step intervention consisting of regulating qi， strengthening the spleen， and warming yang was implemented. The intervention measures included abdominal massage， acupoint application， and hot water bag compress， aiming to regulate qi and unblock the fu-organs， fortify the spleen and stomach to consolidate middle qi， and warm and activate spleen yang.

Results

The discomfort symptoms of gastrointestinal reactions in the pediatric patient were effectively alleviated. Nausea， vomiting， abdominal pain， and diarrhea were significantly relieved， appetite was improved， and the patient demonstrated good compliance with subsequent treatment.

Conclusion

The intervention protocol of abdominal massage combined with acupoint application and hot compress under the guidance of the theory of strengthening the spleen and harmonizing the middle jiao can effectively alleviate gastrointestinal reactions induced by azithromycin. The intervention is safe， simple， and noninvasive with high acceptance among pediatric patients， and is worthy of clinical promotion and application.
