摘要

本文总结1例87岁多病共存高龄老人在三级医院—社区—居家联动模式下的居家康复护理经验。患者共病16种，伴吞咽障碍、认知障碍及重度照护依赖；出院后转介至护理师制定个性化康复护理计划并每周督导，社区护士每周≥3次上门实施吞咽、呼吸、营养、活动及认知康复训练，同时建立快速响应通道。居家1个月期间，患者无因误吸再入院，血氧饱和度维持≥93%，体重从79 kg增至80 kg，活动耐力延长至60 min。该模式通过无缝交接、双向质控与快速响应机制，破解了高龄多病患者“出院即失管”的困局，为同类患者的居家康复护理提供了可复制的实践路径。

Abstract

This article summarizes the home rehabilitation nursing experience of an 87-year-old elderly patient with multiple comorbidities under a tertiary hospital-community-home linkage model. The patient presented with 16 coexisting conditions， accompanied by dysphagia， cognitive impairment， and severe care dependency. Following discharge， the case was referred to transitional care nurses who developed an individualized rehabilitation nursing plan with weekly supervision， while community nurses conducted on-site rehabilitation training targeting swallowing， breathing， nutrition， physical activity， and cognition at least three times per week， along with the establishment of a rapid response channel. During the one-month home rehabilitation period， the patient was not readmitted due to aspiration， oxygen saturation was maintained at ≥93%， body weight increased from 79 kg to 80 kg， and activity tolerance extended to 60 minutes. By ensuring seamless handover， implementing bidirectional quality control， and establishing a rapid response mechanism， this model addresses the common challenge of "loss of follow-up care immediately after discharge" faced by elderly multimorbid patients and provides a replicable practical pathway for home rehabilitation care in similar patients.
