摘要

不寐在中风患者中十分常见，现代医学主要采用镇静催眠类治疗。然而，此类药物改变中风后睡眠障碍患者的正常睡眠结构，且不利于中风病情康复。本案例患者中风后患有继发性癫痫且留置胃管，气管切开，病情复杂，其主要问题为夜间不寐，日夜颠倒，夜间只可间断入睡1 h，匹兹堡睡眠质量指数（PSQI）评分为21分。为此成立多学科病例讨论小组，通过充分的评估及多维度思考、讨论，制定个性化的护理方案并实施。经治疗，患者夜间不寐情况得到改善，夜间可间断入睡5 h，PSQI评分下降至15分。通过本护理案例讨论扩展临床思维，加深中风不寐护理理论知识和实践中医外治法在临床中的应用，取得了良好的疗效，获得患者及家属的高度肯定。

Abstract

insomnia is very common in stroke patients， and modern medicine mainly adopts sedative hypnotic treatment. However， such drugs change the normal sleep structure of patients with sleep disorders after stroke， and are not conducive to the rehabilitation of stroke. The patient in this case suffered from secondary epilepsy after stroke， with gastric tube indwelling and tracheotomy. The condition was complex. The main problem was insomnia at night， day and night reversed， and he could only sleep for 1 h at night. The Pittsburgh sleep quality index （PSQI） score was 21 points. Therefore， a multidisciplinary case discussion group was established to formulate and implement a personalized nursing plan through full evaluation and multi-dimensional thinking and discussion. After treatment， the patient's insomnia at night was improved， and he could sleep intermittently for 5 h at night， and the PSQI score decreased to 15 points. Through the discussion of this nursing case， the clinical thinking was expanded， and the theoretical knowledge of stroke insomnia nursing and the application of TCM External Therapy in clinical practice were deepened， which achieved good curative effect and was highly affirmed by patients and their families.
