摘要

心房颤动是心源性卒中的最常见危险因素，且房颤相关卒中具有高复发率、高致残率及高致死率的特点。本案例旨在通过展示1例房颤所致心源性卒中患者的脑心联合护理过程，由卒中专科与心血管专科组建的联合护理小组，为患者制定并实施个性化诊疗，病情得到有效控制。患者出院时康复效果显著，肢体功能明显改善。为给心源性卒中患者提供更优质全面的护理服务，促进学科发展，脑心联合护理小组组织护理查房活动，介绍病例、化验及辅助检查、病人查体及护理各项评估内容，针对护理问题进行有效的措施干预等，通过本次查房为心源性卒中患者提供多学科、全面、全程的优质护理服务，拓展护士的专业知识，增强专科间的交流与合作，对提升整体护理质量和推动学科发展具有重要意义。

Abstract

Atrial fibrillation is the most common risk factor for cardiogenic stroke， and atrial fibrillation related stroke has the characteristics of high recurrence rate， high disability rate and high mortality. The purpose of this case is to develop and implement personalized diagnosis and treatment for patients by showing the process of brain heart joint nursing of a patient with cardiogenic stroke caused by atrial fibrillation， and the joint nursing team established by stroke specialty and cardiovascular specialty， so that the condition can be effectively controlled. The rehabilitation effect of the patient was significant at discharge， and the limb function was significantly improved. In order to provide better quality and comprehensive nursing services for patients with cardiogenic stroke and promote the development of disciplines， the brain heart joint nursing team organized nursing rounds， introduced cases， laboratory tests and auxiliary examinations， patient physical examination and nursing evaluation contents， and carried out effective measures and interventions for nursing problems. Through this ward round， it provided patients with cardiogenic stroke with multidisciplinary， comprehensive and whole process high-quality nursing services， expanded nurses' professional knowledge， and enhanced exchanges and cooperation between specialties， which was of great significance to improve the overall quality of nursing and promote the development of disciplines.
