摘要

艾滋病合并直肠恶性肿瘤术后肠造口的患者，围手术期CD4淋巴细胞低，免疫力差，缺乏自我照护能力，生活质量难以得到保障，通过四个“1+1”个性化管理模式，对患者进行专业化护理，提高患者生活质量。针对1例艾滋病合并直肠恶性肿瘤术后肠造口患者，进行“医生+护士”一体化查房，MDT诊疗，制定围手术期治疗、护理方案；“线上+线下”搭建交流平台，信息实时共享，促进心理健康；“院内+院外”实施多维度健康宣教及延续护理服务；“门诊+病房”动态造口维护预防并发症。通过四个“1+1”个性化管理模式，对患者围手术期提供专业精细的护理服务，患者的生活质量得到了进一步的提高。

Abstract

Patients with AIDS complicated by rectal malignant tumors who undergo intestinal stoma surgery often face challenges such as low perioperative CD4 lymphocyte levels， compromised immunity， limited self-care capacity， and reduced quality of life. This study implemented a four "1+1" personalized management model to provide specialized nursing care， aiming to enhance the patient's quality of life. For a case involving an AIDS patient with a postoperative rectal malignant tumor and intestinal stoma， the following strategies were applied： 1） "Doctor + Nurse" integrated ward rounds and multidisciplinary team （MDT） collaboration to develop perioperative treatment and nursing plans； 2） "Online + Offline" communication platforms for real-time information sharing and mental health support； 3） "In-hospital + Out-of-hospital" multidimensional health education and continuity of care； 4） "Outpatient + Inpatient" dynamic stoma maintenance to prevent complications. Through this four "1+1" personalized management model， the patient received professional and meticulous perioperative care， resulting in significant improvement in quality of life.
