摘要

本文报告1例多模态中医护理技术治疗中毒性巨结肠（腹压25～28 mmHg，Ⅳ级）患者的个案护理。患者因肠麻痹导致72 h无排便排气，且存在腹腔间隔室综合征高风险。护理团队基于“六腑以通为用”的中医理论，采用多模态中医护理技术（中药灌肠+中药膏摩+耳穴贴压）进行干预。经治疗，患者于第6日恢复排气排便（1～2次/d）；尿动力监测仪显示，24 h后腹压从25～28 mmHg显著降至18～23 mmHg，第7日进一步稳定在11～15 mm Hg。

Abstract

This paper reports a case of multimodal Traditional Chinese Medicine （TCM） nursing interventions for a patient with toxic megacolon （intra-abdominal pressure： 25–28 mmHg， Grade IV）. The patient presented with intestinal paralysis， manifesting as 72 hours of absent defecation and flatus， alongside a high risk of abdominal compartment syndrome. Guided by the TCM theory of "unblocking the six fu organs for functional recovery，" the nursing team implemented a multimodal TCM approach （herbal enema + herbal ointment massage + auricular acupressure）. By the sixth day of treatment， the patient regained daily bowel movements （1–2 times/day）. Urodynamic monitoring revealed a significant reduction in intra-abdominal pressure from 25–28 mmHg to 18–23 mmHg within 24 hours， stabilizing further at 11–15 mmHg by the seventh day.
