摘要

目的

探讨芒硝等中药外敷腹部联合清胰贴穴位敷贴治疗小儿急性胰腺炎的中西医结合护理方法及临床效果。

方法

系统观察1例9岁急性胰腺炎患儿在常规西医治疗基础上，实施中医联合外治法，即芒硝、吴茱萸等中药装入布袋贴敷左上腹，将芒硝、生大黄等中药制成清胰贴，先按摩神阙、天枢及中脘等穴位，再清胰贴精准贴敷至穴位上。于每日9：00实施联合治疗，先清胰贴治疗6 h后取下，再进行芒硝等中药外敷，1次/d，3 d为一疗程，配合中西医结合护理。

结果

ERCP术后治疗72 h，患者VAS评分由4分降至0分，GSRS评分由22分降至17分，血淀粉酶由855 U/L降至87 U/L，过渡至低脂半流质饮食，住院9 d后出院，随访1月无复发。

结论

本研究创新性构建“双途径外治—动态评估—阶梯营养”的中西医结合护理模式，其作用机制是通过芒硝的渗透脱水效应降低胰腺间质压力，配合清胰贴贴敷特定穴位，实现经皮给药与经络传导的双重调节炎症因子水平。

Abstract

Objective

To explore the integrated Chinese-Western nursing methods and clinical efficacy of abdominal external application with Mirabilite and other Chinese herbs combined with acupoint application of Qingyi plaster in treating pediatric acute pancreatitis.

Methods

A systematic observation was conducted on a 9-year-old acute pancreatitis patient. Based on conventional Western therapy， an integrated Chinese-Western external treatment protocol was implemented： Mirabilite and Evodia rutaecarpa were packed into cloth bags and applied to the left upper abdomen， while Qingyi plaster （composed of Mirabilite and raw rhubarb） was precisely applied to acupoints Shenque （CV8）， Tianshu （ST25）， and Zhongwan （CV12） after acupressure. The combined treatment was administered daily at 9：00： Qingyi plaster was applied for 6 hours before removal， followed by Mirabilite-based herbal compress once daily for 3 days as one course， alongside integrated nursing care.

Results

At 72 hours post-ERCP， the patient’s VAS score decreased from 4 to 0， GSRS score improved from 22 to 17， serum amylase level reduced from 855 U/L to 87 U/L， and dietary tolerance advanced to low-fat semi-liquid intake. The patient was discharged after 9 days with no recurrence during 1-month follow-up.

Conclusion

This study innovatively established an integrated nursing model termed "dual-pathway external therapy， dynamic assessment， and stepwise nutrition." Its mechanism involves reducing pancreatic interstitial pressure through the osmotic dehydration effect of Mirabilite， combined with Qingyi plaster’s acupoint application， achieving dual regulation of inflammatory cytokines via transdermal drug delivery and meridian conduction.
