Nursing Experience with Warm Moxibustion and Herbal Hot Compress Combined with Herbal Cream Massage for One Case of Lumbago (Qi Stagnation and Blood Stasis Pattern)
This case report presents an integrated treatment approach for a 37-year-old male patient with lumbago of Qi Stagnation and Blood Stasis pattern. Its academic significance lies in the innovative integration of warm moxibustion and herbal hot compress with herbal cream massage, achieving an organic fusion of traditional external therapies and modern physical treatments. This constructs a precision intervention protocol for Qi Stagnation and Blood Stasis pattern, offering a new paradigm for lumbago diagnosis and treatment systems. The patient presented with low back pain and functional impairment (VAS score: 8/10). Specialized examination revealed reduced lumbar flexion-extension rotation ROM (53% of normal), L4-L5 interspinous tenderness (++), and positive straight leg raise test (45°). Diagnosis of Qi Stagnation and Blood Stasis pattern was confirmed through comprehensive four diagnostic methods.A 6-week treatment protocol (5 sessions/week) was implemented, combining warm moxibustion and herbal permeation techniques with massage stimulation at acupoints including Yaoyangguan (GV3) and Dachangshu (BL25). Multidimensional nursing assessment (physiological-psychological-social domains) guided individualized nursing pathways addressing acute pain management, motor dysfunction, and anxiety (SAS score: 60/100). Key interventions included:Standardized warm moxibustion parameters (42±2℃, 15 min/session), Herbal cream matrix containing Radix Angelicae Sinensis and Carthamus tinctorius for blood-activation, Constant-temperature (37°C) massage particles for thermoconductive therapy, Post-treatment skin care protocols and behavioral guidance (biomechanical protection, activity rhythm adjustment), Post-intervention outcomes demonstrated:VAS score reduced to 1/10, Lumbar ROM restored to 92% of normal, ADL score improved to 98/100, SAS score decreased to 30/100. This study confirms that integrated TCM external therapies exert dual mechanisms through thermodynamic effects and transdermal drug absorption, effectively improving Qi-blood circulation. The biopsychosocial model-based holistic nursing enhances clinical efficacy in chronic pain management. Future multicenter RCTs are recommended to verify long-term outcomes and establish evidence-based standardized clinical pathways.
