摘要

本文总结1例罕见遗传性神经系统疾病脊髓小脑性共济失调3型的特殊临床表现及护理经验。住院期间予患者甲钴胺肌注营养神经，丁苯酞保护线粒体代谢，依达拉奉清除氧自由基，考虑不除外帕金森综合征可能，加用美多芭口服改善患者症状，此外予针刺治疗以补肝益肾，滋阴温阳，配合中药内服、艾条灸及肢体康复等中医综合治疗；出院后，患者症状得到部分改善，改良Rankin量表（mRS）评分为1分。护理框架可为遗传性共济失调患者的个体化治疗提供范式参考，特别在青年起病型病例的早期干预策略制定方面具有重要指导价值。

Abstract

This article summarizes the unique clinical manifestations and nursing experience of a patient with the rare hereditary neurological disorder spinocerebellar ataxia type 3. During hospitalization， the patient received intramuscular mecobalamin for nerve nutrition， butylphthalide for mitochondrial protection， and edaravone for oxygen free radical clearance. Considering possible Parkinsonian syndrome， oral madopar was added to alleviate symptoms. Additionally， comprehensive traditional Chinese medicine interventions were implemented， including acupuncture to tonify liver and kidney， nourish yin and warm yang， combined with herbal decoctions， moxibustion， and limb rehabilitation. Post-discharge， partial symptom improvement was achieved with a modified Rankin Scale （mRS） score of 1. This nursing framework provides a paradigmatic reference for personalized treatment of hereditary ataxia patients， offering significant guidance for developing early intervention strategies， particularly in young-onset cases.
