摘要

回顾性分析1例重症腹腔感染脓毒性休克合并腹部术后切口脂肪液化合并短肠综合征导致困难换药的护理经验。以中医整体观与生物—心理—社会模式为指导制定护理方案，以“急则治标、缓则治本”为原则开展护理措施，提供个性化切口与造口换药方案；采用玉红膏联合银离子敷料协同促进伤口愈合，以改善切口感染及造口失禁性皮炎；使用五音疗法联合艾灸，以缓解焦虑情绪。经过多学科团队协作和精心护理，患者APACHE II评分由36分降至13分，患者恢复良好并于入院92 d后出院。

Abstract

This study retrospectively analyzes the nursing experience of a patient with severe intra-abdominal infection， septic shock， postoperative incisional fat liquefaction， and short bowel syndrome， which complicated wound management. Guided by the holistic principles of Traditional Chinese Medicine （TCM） and the biopsychosocial model， a nursing plan was formulated based on the principle of "treating acute symptoms first and addressing root causes later." Personalized wound and stoma care protocols were implemented， including the combined use of Yuhong Ointment （a TCM preparation） and silver-ion dressings to promote healing， manage wound infection， and alleviate stoma-related incontinence dermatitis. Additionally， five-element music therapy and moxibustion were employed to reduce anxiety. Through multidisciplinary collaboration and meticulous care， the patient's APACHE II score decreased from 36 to 13， with successful recovery and discharge after 92 days of hospitalization.
