Nursing Report on Integrated Chinese and Western Medicine Care for a Case of PICC Catheter-Related Thrombosis
This report summarizes the integrated Chinese and Western medicine nursing experience in managing a case of deep vein thrombosis (DVT) secondary to peripherally inserted central catheter (PICC) placement in a patient with acute lymphoblastic leukemia (ALL). The patient developed right upper limb swelling and pain post-PICC insertion, with ultrasound confirming thrombosis in the mid-distal right subclavian vein, axillary vein, and proximal brachial vein, accompanied by severe hypofibrinogenemia. Western nursing interventions included low-molecular-weight heparin anticoagulation, limb elevation, and topical application of Hirudoid cream, while traditional Chinese medicine (TCM) nursing employed cold compress with Jinhuang San (Golden Yellow Powder). Post-intervention, limb edema resolved without complications such as pulmonary embolism or bleeding. This case highlights the necessity of vigilant thrombosis prevention in hematologic malignancy patients undergoing chemotherapy and demonstrates the synergistic benefits of integrated Chinese-Western nursing in symptom management.
