摘要

本文总结1例糖尿病酮症酸中毒昏迷合并呼吸衰竭气管插管患者的护理经验。患者因胰岛素停用和饮食不规律诱发危象，伴严重代谢紊乱（血糖>41.6 mmol/L）及Ⅱ型呼吸衰竭，需紧急气管插管机械通气。通过西医精准液体复苏、胰岛素泵控糖，联合中医振动排痰、穴位贴敷等技术，实现48 h内成功脱机。重点探讨中西医结合在纠正血容量、改善呼吸功能及心理干预中的协同作用，为危重症患者急救提供差异化护理策略。

Abstract

This article summarizes the nursing experience of a patient with diabetic ketoacidosis （DKA） coma complicated by respiratory failure requiring endotracheal intubation. The patient developed critical symptoms due to insulin discontinuation and irregular diet， accompanied by severe metabolic disturbances （blood glucose >41.6 mmol/L） and type II respiratory failure， necessitating emergency endotracheal intubation and mechanical ventilation. Through precise Western medical interventions such as fluid resuscitation and insulin pump therapy， combined with Traditional Chinese Medicine （TCM） techniques like vibration expectoration and acupoint application， successful extubation was achieved within 48 hours. The study highlights the synergistic effects of integrated Chinese and Western medicine in correcting blood volume， improving respiratory function， and implementing psychological interventions， offering differentiated nursing strategies for critical care management.
