摘要

本文报道1例心肺复苏后并发缺氧缺血性脑病合并脑水肿及细菌性肺炎患者的成功救治经验。通过组建多学科团队实施综合管理，重点开展以下干预措施：意识唤醒与脑缺氧缺血改善治疗，包括阶梯式降颅压药物应用；精细化护理方案，涵盖人工气道管理、肠内营养支持、早期床旁康复训练及情志干预；中医辨证施治，四诊合参诊断为缺血性中风（痰瘀阻络证），予祛痰化瘀中药治疗。经中西医结合干预后，患者颅内压有效控制，呼吸功能逐步恢复并成功脱机，病情趋于稳定。本案例为危重症患者的综合救治提供了可借鉴的临床路径。

Abstract

This article reports the successful treatment of a patient with hypoxic-ischemic encephalopathy complicated by cerebral edema and bacterial pneumonia following cardiopulmonary resuscitation. A multidisciplinary team implemented comprehensive management， focusing on the following interventions： consciousness awakening and cerebral hypoxia-ischemia improvement therapy， including stepwise intracranial pressure reduction with medications； refined nursing protocols encompassing artificial airway management， enteral nutrition support， early bedside rehabilitation training， and emotional intervention； Traditional Chinese Medicine （TCM） syndrome differentiation and treatment， diagnosed as ischemic stroke （phlegm-stasis obstructing collaterals pattern） through four diagnostic methods， with herbal therapy to resolve phlegm and remove stasis. After integrated TCM-Western medicine intervention， the patient achieved effective intracranial pressure control， gradual recovery of respiratory function leading to successful ventilator weaning， and stabilized condition. This case provides a referential clinical pathway for the comprehensive treatment of critically ill patients.
