Case Report on Syndrome-Differentiated Nursing with Heat-Sensitive Moxibustion for an Elderly Patient with Unstageable Pressure Injury
This case report describes a 88-year-old female patient who was bedridden for a long time, with stage IV pressure ulcers on the sacral area (deeply penetrating the periosteum), severe malnutrition (serum albumin 25.2 g/L), incontinence of urine and stool, and dementia. The patient's condition was complex and the nursing risk was extremely high. The nursing team established the core nursing diagnoses of impaired skin integrity, nutritional imbalance, and high risk of infection through comprehensive assessment (ADL score 0, MNA indicated moderate to severe malnutrition, Braden score 8). They then developed an individualized intervention plan. The specific intervention measures are as follows: basic care: strictly implement turning and pressure reduction as well as incontinence management; nutritional support: high-protein enteral nutrition combined with intravenous albumin supplementation; traditional Chinese medicine techniques: innovatively apply heat-sensitive moxibustion combined with wet dressings to promote local blood circulation and tissue repair; emotional care: improve anxiety related to dementia through music therapy and directional sensory stimulation. After 28 days of intervention, the pressure ulcer wound significantly reduced from 9 cm × 14 cm × 3 cm to 6 cm × 9 cm × 1 cm, the exudate and foul smell disappeared, the coverage rate of new granulation tissue reached 80%, and the serum albumin increased to 29.6 g/L, with improvement in nutritional indicators. This case confirms that heat-sensitive moxibustion can enhance the repair efficiency of elderly pressure ulcers through its warming and promoting meridian circulation effect, and the combined treatment model of traditional Chinese and Western medicine has significant clinical value in the management of complex chronic wounds.
