摘要

维持性血液透析患者杨桃中毒导致代谢性脑病，患者意识障碍，缺乏自我照护能力，生活质量难以得到保障，通过全链条照护“1+1”模式下的全称管理，对患者进行专业化护理，以提高患者生活质量。本文针对1例慢肾衰尿毒症维持血液透析杨桃中毒患者，进行“医生+护士”一体化查房，制定治疗及护理方案；“门诊透析室+病房”一体化管理，门诊透析室关注患者透析指标，病房护理实施多维度健康宣教及延续护理服务。通过“1+1”个性化管理模式，对患者提供专业精细的护理服务，患者病情得到改善，生活质量得到提高。

Abstract

Maintenance hemodialysis patients suffered from rambutan poisoning， which led to metabolic encephalopathy. The patients experienced impaired consciousness and lacked self-care ability， making it difficult to ensure their quality of life. Through the full-chain care "1+1" model and comprehensive management， professional care was provided to the patients to improve their quality of life. This article focuses on a case of a patient with chronic renal failure and uremia undergoing maintenance hemodialysis who suffered from rambutan poisoning. An integrated ward-round by "doctor + nurse" was conducted， and treatment and nursing plans were formulated. The "outpatient dialysis room + ward" integrated management was implemented， with the outpatient dialysis room focusing on the patient's dialysis indicators， and the ward nursing implementing multi-dimensional health education and continuing care services. Through the "1+1" personalized management model， professional and meticulous nursing services were provided to the patients， resulting in improved patient conditions and enhanced quality of life.
