Nursing Experience of Improved Blind Manual Placement of Spiral Nasointestinal Tube
This article summarizes the nursing experience of bedside manual blind insertion of a spiral nasoenteric tube combined with a nasogastric tube for nutritional support in a critically ill patient with enteral feeding intolerance. In view of the patient's clinical problems including high gastric residual volume, abdominal distension and pain, emaciation, and poor mental status, a modified tube placement technique was applied on the basis of conventional treatment. To address the challenges of a narrow posterior pharyngeal wall and the occupation of the nasopharyngeal space by the indwelling nasogastric tube, a two-person cooperative method was adopted to facilitate the smooth passage of the catheter through the oropharynx. Optimization of patient positioning (head of bed elevated to 60° and right lateral decubitus position) combined with intermittent pulsed water and air instillation was employed to stimulate pyloric opening. This was further complemented by precise tube advancement and multi-site localization. The modified technique markedly improved the success rate of tube placement, shortened the procedural duration, and avoided the need for patient transfer and equipment dependence. After successful tube placement, the patient's daily gastric drainage volume decreased from 960 mL/d to 100 mL/d on the same day and fell below 20 mL/d within 10 days, with complete resolution of abdominal distension and pain. Enteral nutrition infusion reached the target rate of 60 mL/h. This technique is safe to perform, demonstrates a high success rate, and, when combined with a nasogastric tube, provides an effective nutritional support pathway for patients with enteral feeding intolerance.
