摘要

本文总结1例内瘘穿刺后严重血肿患者经沿经络走形外涂地榆油联合黄连膏湿敷治疗的护理体会。基于中医辨证施护理论，血肿发生后积极进行皮肤血管评估、疼痛评估、肢体活动度评估及心理评估，采用中医经络理念运用地榆油联合黄连膏湿敷，同时结合实施饮食护理、情志护理及肢体功能锻炼等，达到内瘘血肿吸收消散，恢复内瘘功能的目的，为长期血液透析患者发生穿刺意外引起的血肿提供护理临床经验。

Abstract

This article summarizes the nursing experience of treating a severe post-puncture hematoma in an arteriovenous fistula （AVF） patient using topical Sanguisorba oil applied along meridian pathways combined with Coptis ointment wet compress. Guided by TCM syndrome differentiation nursing theory， comprehensive assessments—including skin vascular evaluation， pain assessment， limb mobility， and psychological status—were conducted post-hematoma. The integrated approach， incorporating meridian theory with herbal intervention， dietary guidance， emotional care， and functional exercises， promoted hematoma resolution and restored AVF function. This case provides clinical insights for managing puncture-related hematomas in long-term hemodialysis patients.
