摘要

本文报告1例老年高血压伴焦虑状态患者的个案管理。患者主诉发现血压升高19年，近期出现头胀、乏力等症状。入院后经全面检查，发现患者存在高血压3级（极高危）、胃食管反流、慢性胃炎等多种并发症及焦虑状态。根据患者具体情况，制定个性化的综合治疗方案，包括调整降压药物、改善循环、保护胃黏膜、调节肠道菌群、抗凝治疗及心理治疗等措施。同时，实施包括基础护理、中医特色护理、活动与安全护理、营养与饮食护理以及健康教育与知识普及在内的全面护理计划。经治疗与护理，患者症状得到有效控制，病情相对平稳，出院时需继续服用相关药物并定期随访。

Abstract

This article presents a case report on the management of an elderly patient with hypertension and comorbid anxiety. The patient reported a 19-year history of elevated blood pressure， with recent symptoms including head distension and fatigue. Upon admission， comprehensive examinations revealed grade 3 hypertension （very high risk）， gastroesophageal reflux， chronic gastritis， and anxiety disorder. A personalized integrated treatment plan was developed， incorporating antihypertensive medication adjustment， circulation improvement， gastric mucosal protection， gut microbiota regulation， anticoagulation therapy， and psychological intervention. Additionally， a holistic nursing plan was implemented， encompassing basic care， traditional Chinese medicine （TCM）-specific nursing， activity and safety management， nutritional and dietary guidance， as well as health education. Following treatment and nursing interventions， the patient’s symptoms were effectively controlled， with stabilized condition upon discharge. Long-term medication adherence and regular follow-up were recommended.
