摘要

本文报告1例35岁风痰阻络型面瘫（面神经炎）患者的护理案例。患者因左侧口眼歪斜4 d入院，中医诊断为“口僻（风痰阻络证）”，西医诊断为面神经炎，初始House-Brackmann分级评估为Ⅴ级，伴焦虑（SAS评分66分）、疼痛（VAS评分5分）及睡眠障碍。采用中药膏摩（黄芪、当归、白附子等配伍）联合红外线治疗，辅以面部功能训练、情志护理（五音疗法）及辨证施膳（祛风化痰、健脾化湿饮食）。经10 d治疗，患者面神经功能恢复至Ⅰ级，焦虑、疼痛及睡眠障碍显著改善，中医综合护理方案对风痰阻络型面瘫具有良好疗效，可为临床护理提供参考。

Abstract

This paper reports a nursing case of a 35-year-old patient with facial paralysis （facial neuritis） of wind-phlegm obstructing collaterals type. The patient was admitted with left-sided deviation of the mouth and eye for 4 days， diagnosed as "deviated mouth （wind-phlegm obstructing collaterals pattern）" in Traditional Chinese Medicine （TCM） and facial neuritis in Western medicine. Initial House-Brackmann grading was V， accompanied by anxiety （SAS score 66）， pain （VAS score 5）， and sleep disorders. Treatment included Chinese herbal ointment massage （composed of Astragalus， Angelica sinensis， Typhonii Rhizoma， etc.） combined with infrared therapy， supplemented with facial functional training， emotional nursing （five-element music therapy）， and syndrome-differentiated diet （foods for dispelling wind-phlegm and strengthening spleen to resolve dampness）. After 10 days of treatment， facial nerve function recovered to House-Brackmann grade I， with significant improvement in anxiety， pain， and sleep disorders. The integrated TCM nursing protocol demonstrated efficacy for wind-phlegm obstructing collaterals-type facial paralysis， providing a reference for clinical nursing.
