摘要

本文分析1例急性白血病患者PICC拔管困难的案例，结果表明导管留置时间长、血管痉挛及纤维蛋白鞘形成是主要原因。通过心理疏导、指导手部运动并配合手法按摩后，导管顺利拔除。该护理方法可有效解决PICC拔管困难，避免有创操作，减轻患者负担，对保障医疗安全及提升护理水平具有重要意义。

Abstract

This article analyzes a case of difficult peripherally inserted central catheter （PICC） removal in a patient with acute leukemia. The results indicate that prolonged catheter indwelling time， vascular spasm， and fibrin sheath formation were the primary contributing factors. Through psychological counseling， guided hand exercises， and manual massage techniques， the catheter was successfully removed without invasive intervention. This nursing approach effectively resolves PICC removal difficulties， minimizes patient trauma， and demonstrates significant value for enhancing medical safety and nursing quality.
