摘要

本文总结1例慢性肾衰竭尿毒症期患者采用中药灌肠联合中药饮片治疗以延缓透析的护理实践。患者临床表现为血肌酐反复升高、泡沫尿、周身乏力及下肢水肿加重，中医辨证属脾肾亏虚、湿浊瘀阻。在全面护理评估基础上，通过中药灌肠联合个体化中药饮片的综合干预，患者肾功能损害进展得到有效延缓，临床症状（乏力、水肿）显著改善，顺利出院。

Abstract

This article summarizes the nursing practice of delaying dialysis in a patient with end-stage chronic renal failure through combined herbal enema and individualized Chinese herbal decoction therapy. The patient presented with recurrent elevated serum creatinine， foamy urine， systemic fatigue， and aggravated lower limb edema， which were differentiated in Traditional Chinese Medicine （TCM） as spleen-kidney deficiency with dampness-turbidity and blood stasis. Based on comprehensive nursing assessment， the integrated intervention of herbal enema and tailored herbal decoction effectively slowed the progression of renal impairment， significantly alleviated clinical symptoms （fatigue and edema）， and achieved successful discharge.
