Nursing Experience of Integrated Traditional Chinese and Western Medicine in a Patient with Optic Atrophy Combined with Anterior Ischemic Optic Neuropathy
This study summarizes the nursing practice of a patient with optic atrophy (OA) combined with anterior ischemic optic neuropathy (AION). The patient was a 72-year-old male with a right eye visual acuity of 0.05 and temporal visual field defect, and was also diagnosed with type 2 diabetes and at high risk of falls (Morse score of 50 points). Through systematic assessment, four major nursing problems were identified: altered perception, anxiety, lack of knowledge, and risk of injury. Based on these, an individualized nursing plan centered on "improving vision, controlling risks, and psychological reconstruction" was formulated. On the basis of conventional treatment, a combined traditional Chinese and Western medicine nursing intervention was adopted: Western medicine intervention included vasodilators and neurotrophic drugs; traditional Chinese medicine techniques included acupressure for liver soothing and vision improvement, auricular point sticking to regulate qi and blood, traditional Chinese medicine nebulization for heat-clearing and detoxification, targeted wet compress for promoting meridian circulation, and acupoint massage to enhance circulation. After 25 days of intervention, the patient's right eye visual acuity improved from 0.05 to 0.4; the Barthel index increased from 70 points to 85 points; the Morse fall risk score decreased from 50 points to 30 points (a 40% reduction in risk); and the periorbital blood flow (measured by laser speckle flow imaging) increased from 0.32 mL/min/g to 0.42 mL/min/g (an increase of 31%). This case demonstrates that the combined traditional Chinese and Western medicine nursing approach, through multi-target intervention of "drugs - meridians - functions", can effectively improve the microcirculation of the optic nerve and visual function, providing a replicable individualized nursing plan for elderly patients with OA-AION and metabolic diseases, and has significant clinical reference value.
