摘要

本文总结1例应用集束化护理在高龄重症肺炎患者谵妄中的护理经验。护理过程中采用多学科团队协作模式，通过团队共同研判患者护理问题、提供专业建议以支撑护理方案多维度制定，同时聚焦制定早期康复训练计划、改善营养状况、提升睡眠质量及开展定向刺激四大关键维度实施针对性干预。该集束化干预不仅实现对患者谵妄的精准识别与有效控制，显著减轻谵妄发生及严重程度，还同步提升患者整体护理质量与康复效果，患者病情好转并顺利出院，临床护理效果突出。

Abstract

This article summarizes the nursing experience of applying bundle care to manage delirium in an elderly patient with severe pneumonia. A multidisciplinary team collaboration model was adopted during the nursing process， wherein the team collectively assessed the patient’s care needs and provided professional recommendations to support multidimensional care planning. Interventions focused on four key dimensions： early rehabilitation training， nutritional improvement， sleep quality enhancement， and targeted sensory stimulation. The bundle care approach not only enabled precise identification and effective control of delirium， significantly reducing its occurrence and severity， but also improved overall nursing quality and rehabilitation outcomes. The patient’s condition ameliorated， leading to successful discharge， demonstrating outstanding clinical nursing efficacy.
