摘要

本文通过1例心功能Ⅳ级（NYHA分级）合并Wagner4级糖尿病足（中医诊断为“心水病”“脱疽”）患者的护理实践，探讨心血管内科、内分泌科、外科、皮肤科及伤口学组多学科协作模式的应用效果。通过多学科协作，实施心功能动态监测、精细化血糖调控及规范化创面换药，配合穴位贴敷及保元汤合血府逐瘀汤加减益气活血等个性化干预措施，患者心功能显著改善，NT-proBNP大幅下降，空腹血糖控制达标，创面面积缩小，感染指标明显降低。结果证实，多学科协作可有效阻断“心—足互损”恶性循环，为复杂共病患者的护理提供标准化参考路径。

Abstract

This study presents the nursing management of a patient with NYHA class IV heart failure （"Heart-Water Disease" in traditional Chinese medicine， TCM） and Wagner grade 4 diabetic foot （"Gangrenous Foot" in TCM）， highlighting the efficacy of a multidisciplinary team （MDT） approach involving cardiology， endocrinology， surgery， dermatology， and wound care specialists. Through MDT collaboration， dynamic cardiac function monitoring， precise glycemic control， standardized wound debridement， and personalized interventions （e.g.， acupoint application therapy and modified Baoyuan Decoction combined with Xuefu Zhuyu Decoction for qi replenishment and blood activation） were implemented. The patient demonstrated significant improvements in cardiac function （marked reduction in NT-proBNP levels），达标 fasting glucose， reduced wound area， and normalized infection markers. The results validate that MDT collaboration effectively interrupts the "heart-foot mutual impairment" vicious cycle， offering a standardized reference pathway for nursing complex comorbidities.
