摘要

本文总结1例血液透析患者动静脉内瘘（AVF）疼痛的中西医结合护理经验。针对患者存在的动静脉内瘘疼痛（VAS评分6分）、焦虑（SAS评分65分）及因疼痛终止治疗等问题，在常规护理基础上，采用耳穴贴压（取指、腕、肘、肩穴）、远红外线照射（45 min/次，3次/周）联合干预，同时配合个体化穿刺方案及情志疏导。经上述措施干预后，患者VAS评分降至2分，SAS评分降至40分，未再出现因疼痛终止治疗的情况。结果表明，耳穴贴压联合远红外线照射能调和气血、通经活络，有效缓解AVF疼痛并提升透析治疗耐受性，对改善患者生活质量具有临床意义。

Abstract

This article summarizes the integrated Chinese and Western nursing approach for a hemodialysis patient with arteriovenous fistula （AVF） pain. The patient presented with AVF pain （VAS score 6）， anxiety （SAS score 65）， and treatment interruptions due to pain. In addition to routine care， interventions included auricular acupoint pressure （applied to finger， wrist， elbow， and shoulder points）， far-infrared therapy （45 min/session， 3 sessions/week）， individualized puncture strategies， and emotional counseling. Post-intervention， the patient’s VAS score decreased to 2， SAS score to 40， with no further treatment discontinuation. The results demonstrate that auricular acupoint pressure combined with far-infrared therapy can regulate qi and blood， promote meridian circulation， effectively alleviate AVF pain， enhance dialysis tolerance， and improve quality of life， highlighting its clinical significance.
