摘要

本文总结1例高龄脑卒中患者因外周留置针输注中/长链脂肪乳导致外渗的病例。通过综合评估患者全身情况，根据肢体肿胀及疼痛分级，采用刺血拔罐缓解局部皮肤及皮下组织张力；待张力释放后，联合水胶体辅料保护皮肤，并持续监测患处臂围、渗出面积、疼痛评分、皮肤颜色、肿胀程度、毛细血管充盈及皮肤完整性变化。经5 d干预后，患者肢体功能基本恢复，本案例旨在为临床处理类似情况提供实践参考。

Abstract

This article summarizes the case of an elderly stroke patient who experienced extravasation of medium/long-chain fat emulsion during peripheral intravenous catheter infusion. Through comprehensive assessment of the patient’s systemic condition and based on the grading of limb swelling and pain， bloodletting cupping was applied to alleviate local skin and subcutaneous tissue tension. After tension release， hydrocolloid dressing was used to protect the skin， while continuous monitoring was conducted on the affected limb’s circumference， extravasation area， pain score， skin color， swelling severity， capillary refill， and skin integrity. After 5 days of intervention， the patient’s limb function was nearly restored. This case aims to provide practical guidance for managing similar clinical scenarios.
