摘要

本文总结1例高龄重症溃疡性结肠炎伴重度营养不良患者的营养护理经验。护理核心要点包括：①营养筛查与评估：精准计算每日营养需求，为制定个性化营养方案提供依据；②早期肠内营养支持：启动肠内营养，以维护肠道屏障功能、改善营养状况；③适时联合肠外营养：及时补充肠外营养，以维持水电解质及氮平衡；④精准病情监测：每日精确记录排便次数、性状及量，动态评估肠道耐受情况。经过30 d积极治疗与精细化护理，患者病情得到显著控制，营养指标明显改善，体力恢复可下床活动。

Abstract

This article summarizes the nutritional care experience of a 75-year-old patient with severe ulcerative colitis and severe malnutrition. The core points of nursing include： ① Nutritional screening and assessment： Accurately calculate the daily nutritional requirements to provide a basis for formulating individualized nutritional plans； ② Early enteral nutrition support： Initiate enteral nutrition to maintain intestinal barrier function and improve nutritional status； ③ Timely combined parenteral nutrition： Timely supplement parenteral nutrition to maintain water， electrolyte and nitrogen balance； ④ Precise condition monitoring： Daily accurately record the frequency， nature and volume of defecation， and dynamically assess intestinal tolerance. After 30 days of active treatment and meticulous care， the patient's condition was significantly controlled， nutritional indicators improved significantly， and physical strength recovered to the point where they could get out of bed and move around.
