摘要

本文总结1例失能老年患者局部皮损面积大于50 cm

的重度失禁相关性皮炎（IAD）的中西医结合护理经验。通过制定结构化皮肤护理方案，实施全方位精细化护理干预，结合隔姜灸疗法与饮食调护改善二便功能，同时加强照护者宣教等综合措施，患者会阴部皮肤状况评分及IAD皮肤损伤评分均显著下降。该案例为同类患者的中西医结合护理实践提供有益参考。

Abstract

This article summarizes the integrated traditional Chinese and Western medicine nursing experience for a bedridden elderly patient with severe incontinence-associated dermatitis （IAD） involving localized skin lesions exceeding 50 cm². By implementing a structured skin care protocol， delivering comprehensive and meticulous nursing interventions， combining ginger-partitioned moxibustion therapy and dietary adjustments to improve bowel/bladder function， and reinforcing caregiver education， the patient’s perineal skin condition score and IAD severity score significantly decreased. This case provides valuable insights for the nursing management of similar patients using an integrated approach.
