摘要

本文对1例慢性肾衰竭血液透析并发肛周脓肿患者的中西医结合治疗及针对性临床护理干预进行回顾性分析。根据患者症状、既往病史及诊断结果，西医予莫匹罗星软膏外用（3次/d，抗感染消肿）、碳酸钙片、碳酸司维拉姆片及环硅酸锆钠散等基础治疗；中医辨证为“肛痈（湿热毒蕴证）”“慢性肾衰病（脾肾亏虚、湿浊瘀阻证）”，以补脾益肾、利湿化浊、清热解毒为治则，予厚朴七物汤合四君子汤加减的中药汤剂口服，1剂/d，分2次温服，同时配合中药坐浴（水温38～40℃，1次/d，以清热解毒、消肿止痛）。结果表明，中西医结合治疗联合专项护理，可有效缓解患者肛周疼痛、促进脓肿创面愈合，同时改善患者营养状况及透析间期生活质量，可为合并慢性肾衰竭血液透析基础疾病的肛周脓肿患者提供临床治疗与护理参考。

Abstract

This article retrospectively analyzes the integrated traditional Chinese and Western medicine treatment and targeted clinical nursing intervention for a patient with chronic renal failure undergoing hemodialysis complicated with perianal abscess. Based on the patient's symptoms， past medical history， and diagnostic results， Western medicine treatment included topical application of mupirocin ointment （3 times/day for anti-infection and swelling reduction）， calcium carbonate tablets， sevelamer carbonate tablets， and sodium zirconium cyclosilicate powder for basic treatment. Traditional Chinese medicine diagnosed the patient with "anal abscess （wet-heat-toxin accumulation syndrome）" and "chronic renal failure （spleen-kidney deficiency， dampness-turbidity and blood stasis syndrome）". The treatment principle was to tonify the spleen and kidney， promote diuresis and remove turbidity， and clear heat and detoxify. The patient was prescribed a modified decoction of Houpo Qiwu Decoction combined with Sijunzi Decoction， taken orally once a day， divided into two doses， and warm. At the same time， traditional Chinese medicine sitz baths （water temperature 38-40℃， once a day） were used to clear heat and detoxify， reduce swelling and relieve pain. The results showed that the combination of integrated traditional Chinese and Western medicine treatment and specialized nursing could effectively relieve perianal pain， promote the healing of abscess wounds， and improve the patient's nutritional status and quality of life during the interdialytic period. It can provide clinical treatment and nursing references for patients with perianal abscess complicated with chronic renal failure undergoing hemodialysis.
