摘要

本文报告1例70岁肺癌男性患者行胸腔镜肺叶切除术后并发广泛皮下气肿（累及颜面部、胸腹壁及阴囊）的全程护理过程。在西医常规护理基础上，配合胸腔闭式引流、皮下穿刺排气等针对性措施，同步采用200 g芒硝纱布袋外敷（每6 h更换1次）联合耳针疗法的中医特色护理模式，同时开展呼吸康复训练、心理疏导等综合干预。该方案有效改善患者的疼痛及焦虑状态，为肺癌术后并发症的中西医结合管理提供临床护理实践参考。

Abstract

This article reports the whole nursing process of a 70-year-old male lung cancer patient who developed extensive subcutaneous emphysema involving the facial region， chest and abdominal wall as well as scrotum after thoracoscopic lobectomy. Based on conventional Western medical nursing， targeted measures such as closed thoracic drainage and subcutaneous puncture for exhausting were carried out. Meanwhile， characteristic traditional Chinese medicine nursing models including external application of 200 g mirabilite wrapped in gauze （changed every 6 h） combined with auricular acupuncture therapy were adopted， together with comprehensive interventions including respiratory rehabilitation training and psychological counseling. This protocol effectively relieved the patient's pain and anxiety， providing clinical nursing practice reference for integrated Chinese and Western medicine management of postoperative complications of lung cancer.
