Nursing Report on Workplace-Adapted Fragmentary Baduanjin Intervention for a Sedentary Worker with Pain-Fatigue Syndrome
This study innovatively implemented a fragmented modified Baduanjin intervention program for a 32-year-old female radiological technician with prolonged sitting-related pain-fatigue syndrome. The patient had only temporary relief of symptoms through conventional massage and physical therapy (once every 2 weeks for a total of 8 sessions), and the symptoms recurred within 1 hour after returning to work, causing back pain and fatigue. Based on the traditional Chinese theory of "combination of movement and stillness" and the modern workplace health management needs, the nursing team broke down the traditional Baduanjin into three sets of scenario-based micro-acts: ① In the workplace scenario, "seated posture supporting the sky" (modified from "both hands supporting the sky to regulate the trunks") was used once per hour to stretch the spine; ② During the meeting break, "standing posture arching the bow" (adapted from "left and right arching like shooting an archer") was completed with elastic bands to strengthen the stability of the scapula; ③ During the lunch break, "lying on back rolling" (optimized from "both hands supporting the feet to stabilize the kidneys and waist") was performed through hip joint flexion and extension to relieve the lumbar muscles. The patient practiced a total of 20-25 times per day, and simultaneously integrated ergonomic sitting posture correction (with a 15° tilt of the lumbar support pad) and emotional regulation (performing 3 rounds of 5-minute mindfulness breathing daily). After 8 weeks of intervention, the assessment showed that the pain visual analogue scale (VAS) score decreased from 6 points to 3 points, the total score of the multidimensional fatigue scale (MFI-20) decreased from 18 points to 10 points, and the average daily effective working time of the patient increased from 2 hours to 5 hours. This practice confirmed that by embedding "high-frequency and short-duration" traditional Chinese guidance scenarios, it can break through the limitation of the separation between traditional rehabilitation treatment and workplace scenarios, providing a combined Chinese and Western medicine nursing practice path for workplace chronic musculoskeletal pain and fatigue management.
