A Case Report on Integrated Traditional Chinese and Western Medicine Nursing for Enhanced Recovery After Surgery in Acute Appendicitis
This paper presents the integrated Traditional Chinese and Western medicine nursing intervention for a 45-year-old male patient with acute appendicitis complicated by intestinal adhesions discovered during surgery. Nursing assessment was based on the Traditional Chinese Medicine (TCM) syndrome differentiation of "Dampness-Heat Accumulation in the Intestines" and the Enhanced Recovery After Surgery (ERAS) concept, identifying three core nursing issues: pain management, gastrointestinal function recovery, and emotional regulation. Interventions included auricular acupressure (Shenmen and Sympathetic points) combined with PCA pump analgesia, herbal patch application (Rhubarb and Mirabilite) at the Shenque acupoint to promote peristalsis, Zusanli acupoint massage, and syndrome-tailored diet therapy (Coix Seed Porridge). Outcomes demonstrated significant improvements: earlier first postoperative flatus (14 hours post-surgery, approximately 10 hours ahead of conventional recovery), reduced pain scores (VAS decreased from 3 to 1), and alleviated anxiety (SAS score declined from 52 to 38). The integrated nursing model showed marked advantages in promoting recovery for overweight patients with intestinal adhesions, offering replicable clinical insights.
