摘要

总结1例体表面积剥脱＞90%TBSA的中毒性表皮坏死症（Toxic Epidermal Necrolysis，TEN）患者的系统化护理经验。在多学科协作（MDT）基础上，对1例27岁女性极危重患者实施前瞻性系统性护理，内容包括分阶段个体化创面管理，多模式疼痛管理，严格的感染防控，个体化营养支持，全程心理支持。经20 d系统干预，患者全身表皮新生面积达体表面积的30%TBSA，感染指标恢复正常，疼痛及营养状况显著改善，顺利转入烧伤科继续治疗。实践证明，对极危重TEN患者实施前瞻性、系统化的护理是提高救治成功率、改善预后的关键。

Abstract

This paper summarizes the systematic nursing experience of a patient with Toxic Epidermal Necrolysis （TEN） involving >90% total body surface area （TBSA） epidermal detachment. Based on multidisciplinary team （MDT） collaboration， prospective and systematic nursing was implemented for a 27-year-old critically ill female patient， including phased individualized wound management， multimodal pain management， strict infection control， personalized nutritional support， and full-course psychological support. After 20 days of systematic intervention， the patient achieved 30% TBSA epidermal regeneration， normalized infection indicators， significantly improved pain and nutritional status， and was successfully transferred to the burn department for further treatment. Practice has shown that prospective and systematic nursing for critically ill TEN patients is crucial to improving treatment success rates and prognosis.
