摘要

本文总结1例高龄急性呼吸衰竭合并急性胆囊炎患者的中西医结合护理体会。针对该患者病情特点（诱因复杂、进展迅速、危重程度高），护理重点包括快速识别呼吸衰竭与胆囊炎诱因并动态评估病情，立即实施抢救以纠正低氧血症；尊重患者及家属选择保守治疗（拒绝气管插管、胆囊穿刺引流及ERCP）的意愿，制定个体化护理方案。护理措施涵盖急救护理、感染预防、呼吸道管理、氧疗支持、营养支持、管路护理及中医特色护理。经积极救治，患者生命体征平稳后转至急诊留观病房继续对症治疗，经个性化干预后症状显著改善。

Abstract

This article summarizes the experience of combined traditional Chinese and Western medicine nursing for a patient with advanced age， acute respiratory failure and acute cholecystitis. Considering the patient's condition characteristics （complex triggers， rapid progression， high severity）， the key points of nursing included rapid identification of the triggers of respiratory failure and cholecystitis and dynamic assessment of the condition， immediate implementation of rescue measures to correct hypoxemia； respecting the patient's and family's choice of conservative treatment （refusal of tracheal intubation， cholecystic puncture drainage and ERCP）， and formulating individualized nursing plans. The nursing measures covered emergency care， infection prevention， respiratory tract management， oxygen therapy support， nutritional support， pipeline care and traditional Chinese medicine characteristic care. After active treatment， the patient's vital signs were stable and then transferred to the emergency observation ward for continued symptomatic treatment. After personalized intervention， the symptoms significantly improved.
