摘要

本文总结1例90岁高龄居家失能4期压力性损伤患者的护理经验。采用“中医整体调护+家庭协作”模式，结合伤口评估三角开展结构化伤口护理，依据中医疮疡理论分期，实施火龙罐综合灸、穴位按摩及五音疗法，同时培训家属照护能力。经19 d居家干预，患者创面完全愈合，纳食量增加，关节僵硬改善；疼痛及照护者焦虑情绪缓解，且照护者掌握规范照护技能。实践表明，该模式可有效改善高龄失能患者压力性损伤预后，适合居家推广。

Abstract

This article summarizes the nursing experience of a 90-year-old elderly disabled patient with stage 4 pressure injury who lived at home. The "traditional Chinese medicine holistic care + family collaboration" model was adopted， combined with the wound assessment triangle to carry out structured wound care. According to the theory of ulcers in traditional Chinese medicine， comprehensive moxibustion with fire dragon cans， acupoint massage and five-tone therapy were implemented. At the same time， family caregiving skills were trained. After 19 days of home intervention， the patient's wound completely healed， the food intake increased， joint stiffness improved； pain and the anxiety of caregivers were relieved， and caregivers mastered the standard caregiving skills. The practice shows that this model can effectively improve the prognosis of pressure injuries in elderly disabled patients and is suitable for promotion in the home setting.
