Nursing Based on Syndrome Differentiation for a Patient with Post-Stroke Sequelae Complicated by Hypertension, Atrial Fibrillation, and Cervical Spondylosis
To explore the clinical value of integrated traditional Chinese and Western medicine (TCM-WM) nursing in elderly patients with multimorbidity, this study focused on a 65-year-old patient with post-stroke sequelae, hypertension, atrial fibrillation, and cervical spondylosis. Core nursing issues were identified through Western functional assessments (Barthel Index, Morse Fall Risk Scale) and TCM four-diagnosis integration (syndrome differentiation: wind-phlegm obstructing collaterals). A coordinated TCM-WM nursing plan was designed and implemented based on the principle of "Western disease diagnosis + TCM syndrome differentiation." After a 2-week intervention and 30-day follow-up, the patient showed significant improvement: neck/shoulder pain resolved (VAS score decreased from 2 to 0), self-care capacity markedly enhanced (Barthel Index increased from 61 to 95), fall risk reduced to low (Morse score declined from 45 to 15), no nursing complications occurred, and TCM symptoms were alleviated. The results demonstrate that TCM-WM nursing effectively improves clinical symptoms and quality of life in elderly patients with post-stroke sequelae and multiple comorbidities, offering a practical approach for managing such populations.
