摘要

探索以5A模式为护理框架，联合中药热奄包与耳穴疗法的中西医结合护理方案，为前列腺增生术后患者提供个性化护理服务。本文选取1例良性前列腺增生（BPH）术后患者，以评估（Assessment）、建议（Advice）、达成共识（Agreement）、协助（Assistance）、随访（Arrangement）5A模式为核心实施护理干预，通过多维度评估明确患者的临床症状、健康风险及中医辨证结果，基于评估结果制定个性化中西医结合护理计划，同步落实西医基础护理与中医特色护理措施，并对患者进行持续随访。干预后患者康复效果显著，可自主排尿且无尿失禁发生，日常生活实现基本自理，全程未出现相关并发症，护理满意度达100%。该护理方案可系统改善BPH患者经尿道前列腺电切术（TURP）术后的疼痛、排尿障碍等问题，降低并发症发生风险，具有一定的临床应用价值，后续需扩大样本量进一步验证其有效性与普适性。

Abstract

This study explores a combined traditional Chinese and Western medicine nursing plan based on the 5A model， integrating traditional Chinese medicine hot compress and auricular therapy， to provide personalized nursing services for patients after benign prostatic hyperplasia （BPH） surgery. A case of a patient who underwent transurethral resection of the prostate （TURP） for BPH was selected to evaluate the effectiveness of the nursing intervention. The 5A model， which includes assessment， advice， agreement， assistance， and arrangement， was used as the core framework. Through multi-dimensional assessment， the patient's clinical symptoms， health risks， and TCM syndrome differentiation results were clearly identified. Based on the assessment results， a personalized combined traditional Chinese and Western medicine nursing plan was formulated， and both Western medicine basic care and TCM characteristic care measures were implemented simultaneously. The patient was also followed up continuously. After the intervention， the patient's recovery was significant， with the ability to urinate independently and no occurrence of urinary incontinence. The patient achieved basic self-care in daily life， and no related complications occurred throughout the process. The nursing satisfaction rate was 100%. This nursing plan can systematically improve postoperative pain and urinary dysfunction in BPH patients after TURP， reduce the risk of complications， and has certain clinical application value. Further validation of its effectiveness and universality is needed by expanding the sample size in the future.
