摘要

目的

探讨中西医结合方案治疗肺炎合并抗生素相关性腹泻（AAD）的临床疗效与安全性。

方法

回顾性分析1例肺炎患者在抗生素抗感染治疗期间并发AAD的临床资料，西医予调整抗生素种类、补液纠正电解质紊乱、微生态制剂调节肠道菌群等基础治疗；中医辨证为脾胃虚弱、寒湿困脾，予健脾祛湿、和胃止泻为法的中药汤剂口服，配合温灸、中药热奄包、穴位按摩、中药外敷辅助调理，并在用药护理、饮食护理、情志护理等方面给予指导。

结果

治疗14 d后，患者腹泻症状完全缓解，肺炎感染指标降至正常，于2025年10月02日出院。

结论

针对肺炎合并AAD患者，在西医规范治疗基础上，结合中医辨证施治及外治法，可有效改善肠道功能、缩短病程，提升整体治疗效果，且安全性良好，值得临床参考。

Abstract

Objective To explore the clinical efficacy and safety of an integrated traditional Chinese and Western medicine treatment plan for pneumonia complicated with antibiotic-associated diarrhea （AAD）.

Methods

A retrospective analysis was conducted on the clinical data of a patient with pneumonia who developed AAD during antibiotic anti-infection treatment. Western medicine treatment included adjusting the type of antibiotics， fluid replacement to correct electrolyte imbalance， and using microecological preparations to regulate the intestinal flora. Traditional Chinese medicine treatment was based on the diagnosis of spleen and stomach weakness and cold-dampness trapping the spleen， with oral administration of a decoction to strengthen the spleen， remove dampness， harmonize the stomach， and stop diarrhea. It was also supplemented with moxibustion， hot compress with traditional Chinese medicine， acupoint massage， and external application of traditional Chinese medicine for auxiliary treatment. Guidance was provided in medication care， dietary care， and emotional care.

Results

After 14 days of treatment， the patient's diarrhea symptoms were completely relieved， and the pneumonia infection indicators returned to normal. The patient was discharged on October 2， 2025.

Conclusion

For patients with pneumonia complicated with AAD， on the basis of standardized Western medicine treatment， the combination of traditional Chinese medicine syndrome differentiation and treatment and external therapies can effectively improve intestinal function， shorten the course of the disease， enhance the overall treatment effect， and has good safety， which is worthy of clinical reference.
