摘要

目的

探讨甲状腺滤泡癌的临床特征及诊断治疗策略。

方法

回顾性分析1例80岁女性患者病例，影像学提示左甲状腺区巨大囊实混合性占位伴双肺多发转移；经超声造影、弹性成像检查及多学科会诊评估后，予全甲状腺切除术治疗，术后病理确诊为甲状腺滤泡癌，后续辅以碘131放射治疗。

结果

患者术后恢复良好，随访显示治疗有效，凸显超声造影技术在疑难病例诊断中的应用价值。

结论

甲状腺滤泡癌虽罕见且侵袭性强，但通过规范化围手术期管理及综合治疗可取得良好疗效，该诊疗模式对高龄合并早期血行转移的病例具有重要指导意义。

Abstract

Objective

To investigate the clinical characteristics， diagnostic approaches， and therapeutic strategies for follicular thyroid carcinoma （FTC）.

Methods

A retrospective analysis was conducted on an 80-year-old female patient presenting with a giant cystic-solid mass in the left thyroid region and multiple bilateral pulmonary metastases， as revealed by imaging. Multidisciplinary evaluation， including contrast-enhanced ultrasound and elastography， guided total thyroidectomy， with postoperative pathology confirming FTC. Subsequent adjuvant therapy involved radioactive iodine-131 treatment.

Results

The patient achieved favorable postoperative recovery， and follow-up demonstrated therapeutic efficacy， highlighting the diagnostic utility of contrast-enhanced ultrasound in complex cases.

Conclusion

Although FTC is rare and aggressive， standardized perioperative management and multimodal therapy can yield positive outcomes. This diagnostic and therapeutic framework holds significant implications for elderly patients with early hematogenous metastasis.
