摘要

本研究基于“清热利湿、益气养阴”理论，采用头部全息刮痧联合中药热熨敷方案，治疗1例虚实夹杂型癃闭（西医诊断前列腺增生）患者。该患者因“尿频、尿急、腹胀3年余，近1月加重”就诊，伴腹胀数字评价量表（NRS）评分6分、匹兹堡睡眠质量指数（PSQI）18分、焦虑自评量表（SAS）55分，严重影响生活质量。干预措施包括一般护理、西医对症支持治疗及子午流注情志护理，经治疗后患者腹胀、睡眠及焦虑症状显著改善，各项评分均降至正常范围，排尿功能恢复。本案例证实该中医特色方案疗效确切，建议扩大样本量进一步明确其临床应用价值。

Abstract

Based on the theory of "clearing heat， promoting diuresis， tonifying qi and nourishing yin"， this study adopted a combined therapy of holistic scalp scraping and hot compress with Chinese herbs to treat a patient with mixed deficiency and excess syndrome of enuresis （diagnosed as benign prostatic hyperplasia in Western medicine）. The patient presented with symptoms of frequent urination， urgency， and abdominal distension for over three years， with a recent aggravation in the past month. The patient had a Numerical Rating Scale （NRS） score of 6 for abdominal distension， a Pittsburgh Sleep Quality Index （PSQI） score of 18， and a Self-Rating Anxiety Scale （SAS） score of 55， which significantly affected the quality of life. The intervention measures included general nursing， Western medicine symptomatic and supportive treatment， and emotional nursing based on the theory of Ziwu Liuzhu. After treatment， the patient's abdominal distension， sleep， and anxiety symptoms were significantly improved， and all scores dropped to the normal range. The patient's urination function was restored. This case confirmed the definite therapeutic effect of this traditional Chinese medicine （TCM） characteristic treatment plan. It is suggested to expand the sample size to further clarify its clinical application value.
