A Case Report on the Nursing Care of a Patient with Liver Depression-Type Insomnia Treated with Holistic Scraping Technique
This report focuses on the nursing practice of a 70-year-old female patient with liver depression-type insomnia, exploring the systematic application of holistic scraping technique and the integrated nursing model in this specific syndrome. The case is unique as the patient is an elderly woman with poor response to conventional Western medicine (estazolam) and insomnia closely related to emotional factors, which is consistent with the liver depression syndrome in traditional Chinese medicine. It provides a typical sample for observing the clinical value of non-pharmacological therapies in patients with refractory and clearly defined syndrome-type insomnia. The main body of the case report centers on the comprehensive nursing plan based on holistic scraping technique. After the intervention, the Pittsburgh Sleep Quality Index (PSQI) score decreased from 17 to 6, the Insomnia Severity Index (ISI) score dropped from 19 to 10, the Visual Analogue Scale (VAS) score for headache decreased from 7 to 1, and the Self-Rating Anxiety Scale (SAS) score reduced from 56 to 35. Follow-up showed stable efficacy, and the patient had stopped using sleep medication. This case confirms that for patients with liver depression-type insomnia, especially the elderly, those with poor response to Western medicine, or those concerned about drug dependence, the holistic scraping technique guided by syndrome differentiation combined with structured nursing is a safe, effective, and well-received non-pharmacological intervention approach.
