摘要

目的

探讨中西医结合护理在老年重度吸入性肺损伤患者中的临床应用效果，为该类患者的护理提供循证依据与实践方案。

方法

对1例87岁因燃烧被褥烟雾吸入导致重度吸入性肺损伤、合并急性呼吸窘迫综合征（ARDS）及Ⅱ型呼吸衰竭的男性患者，实施为期36 d的中西医结合系统化护理方案。

结果

干预后患者气道分泌物由黑灰色黏液转为白色稀薄痰，氧合指数从108 mmHg升至304 mmHg，成功脱离呼吸机并拔除气管切开套管，住院期间未发生压疮、深静脉血栓等并发症。

结论

中西医结合护理通过多靶点协同干预，可显著改善老年重度吸入性肺损伤患者的预后，其动态辨证与精准干预模式可为临床护理实践提供参考。

Abstract

Objective

To explore the clinical efficacy of integrated traditional Chinese and Western medicine （TCM-WM） nursing in elderly patients with severe inhalation lung injury， providing evidence-based rationale and practical protocols for such cases.

Methods

A systematic 36-day TCM-WM nursing intervention was implemented for an 87-year-old male patient with severe inhalation lung injury （caused by smoke inhalation from burning bedding）， complicated by acute respiratory distress syndrome （ARDS） and type II respiratory failure.

Results

Post-intervention， the patient’s airway secretions transitioned from black-gray mucus to white thin sputum； the oxygenation index improved from 108 mmHg to 304 mmHg； successful weaning from mechanical ventilation and decannulation of the tracheostomy tube were achieved. No complications （e.g.， pressure ulcers or deep vein thrombosis） occurred during hospitalization.

Conclusion

Integrated TCM-WM nursing， through multi-target synergistic interventions， significantly improved the prognosis of this elderly patient with severe inhalation lung injury. The dynamic syndrome differentiation and precision intervention model may serve as a reference for clinical nursing practice.
