摘要

本文报告1例中风后失眠合并后循环缺血老年患者的护理过程。患者因阵发性头晕入院，存在夜间躁动不安、昼夜颠倒等严重睡眠障碍。在常规治疗基础上，采用耳部刮痧联合耳穴压丸的中医外治方案进行干预。经近4周治疗，患者夜间躁动的发作频率与强度显著减轻，匹兹堡睡眠质量指数（PSQI）评分下降，头晕、心悸等症状亦得到缓解。结果表明，对此类心脑同病、本虚标实的患者，耳部刮痧联合耳穴压丸可调节脏腑神机，有效改善睡眠及核心症状，该非药物干预方案安全可行，具有临床参考价值。

Abstract

This paper reports the nursing process of an elderly patient with post-stroke insomnia complicated by posterior circulation ischemia. The patient was hospitalized due to paroxysmal dizziness and exhibited severe sleep disturbances， including nocturnal agitation and day-night reversal. Based on conventional treatment， an integrated external therapy of ear scraping and auricular acupoint bead application was implemented. After nearly 4 weeks of intervention， the frequency and intensity of nocturnal agitation significantly decreased， the Pittsburgh Sleep Quality Index （PSQI） score improved， and symptoms such as dizziness and palpitations were alleviated. The results suggest that for patients with concurrent cardiovascular and cerebral disorders presenting as deficiency-rooted excess patterns， this combined TCM approach can regulate visceral functions and mental activity， effectively improving sleep and core symptoms. The non-pharmacological intervention is safe and feasible， offering clinical reference value.
