摘要

本文总结预见性护理联合精细化体位管理在预防高龄髋关节置换手术患者术中压力性损伤中的应用与效果。患者为93岁拟行人工股骨头置换术的超高危病例。手术室护理团队实施预见性护理策略，术前完成全面风险评估与皮肤准备，术中执行个体化、精细化侧卧位摆放管理，综合应用高级减压垫与重点部位加强防护等措施。经全面干预，患者手术顺利，术毕安返ICU，全身皮肤完整，未发生压力性损伤。实践证明，针对高龄、高风险髋关节置换手术患者，手术室团队通过实施预见性护理并执行精细化、标准化体位管理方案，二者联合能够有效阻断术中压力性损伤的发生链，是保障患者手术安全的关键护理措施。

Abstract

This article summarizes the application and efficacy of prophylactic nursing combined with precise positioning management in preventing intraoperative pressure injury in an elderly patient undergoing hip arthroplasty. The patient was a 93-year-old ultra-high-risk case scheduled for artificial femoral head replacement. The operating room nursing team implemented proactive nursing strategies， including comprehensive preoperative risk assessment and skin preparation， as well as individualized and meticulous lateral positioning management during surgery， integrating advanced pressure-relieving pads and reinforced protection for high-risk areas. Through these interventions， the procedure was completed successfully， with the patient returning to the ICU postoperatively with intact skin and no occurrence of pressure injury. The results demonstrate that for elderly， high-risk hip arthroplasty patients， the combined application of prophylactic nursing and standardized， precise positioning protocols by the surgical team effectively disrupts the chain of intraoperative pressure injury， serving as a critical nursing measure to ensure surgical safety.
