摘要

本研究报告1例以间断腹胀伴便血为主要表现的混合痔（脾胃虚证）合并贫血患者的中西医结合治疗与护理过程。患者经保守治疗效果不佳，行痔切除术+经肛门镜内痔套扎术，围手术期采用中药外洗熏浴、中药热奄包及神阙穴外敷等中医外治技术，联合西医创面换药、止痛、止血及促排便等综合干预。结果显示，患者术后恢复平稳，肛周疼痛、水肿及湿疹样改变逐步改善，便血症状消失，顺利出院。本病例围绕痔病术后创面修复与中医辨证施护，探讨中药外治在混合痔围手术期的应用价值，为同类病例提供护理参考。

Abstract

This case report describes the integrated traditional Chinese and Western medicine treatment and nursing process for a patient with mixed hemorrhoids （spleen-stomach deficiency pattern） complicated by anemia， presenting with intermittent abdominal distension and hematochezia. As conservative treatment proved ineffective， the patient underwent hemorrhoidectomy combined with trans-anal anoscope hemorrhoidal ligation. During the perioperative period， traditional Chinese medicine （TCM） external therapies were applied， including TCM herbal fumigation and washing， hot herbal compress， and Shenque （CV8） acupoint application， alongside comprehensive Western medicine interventions such as wound dressing changes， analgesia， hemostasis， and bowel movement promotion. The patient recovered uneventfully after surgery， with gradual alleviation of perianal pain， edema， and eczematous changes， complete resolution of hematochezia， and smooth discharge. Centered on postoperative wound repair and TCM syndrome differentiation-based nursing， this case explores the application value of TCM external treatment in the perioperative management of mixed hemorrhoids and provides a nursing reference for similar cases.
