摘要

本文总结1例67岁女性腰椎管狭窄症患者行腰椎单侧双通道脊柱内镜（UBE）手术的围术期护理经验。针对患者术前焦虑、睡眠障碍及术后疼痛等问题，采用耳穴疗法个体化干预融入加速康复外科（ERAS）路径全程管理模式，辅以规范化镇痛与早期康复训练，出院后实施延续性护理，实现院内到居家的无缝管理。结果显示，患者匹兹堡睡眠指数（PSQI）、焦虑自评量表（SAS）评分、视觉模拟疼痛评分（VAS）均显著改善。本个案总结表明，耳穴疗法融入ERAS路径可有效缓解腰椎UBE围术期应激反应，改善疼痛、焦虑与睡眠状况，加速患者术后康复并提升自我管理能力。

Abstract

This paper summarizes the perioperative nursing experience for a 67-year-old female patient with lumbar spinal stenosis who underwent unilateral biportal endoscopy （UBE） surgery. In response to the patient's preoperative anxiety， sleep disturbance， and postoperative pain， an individualized auricular acupressure intervention was integrated into the entire Enhanced Recovery After Surgery （ERAS） pathway management model， supplemented by standardized analgesia and early rehabilitation training， with continuous nursing implemented after discharge to achieve seamless transition from hospital to home care. The results demonstrated significant improvements in the patient's Pittsburgh Sleep Quality Index （PSQI）， Self-Rating Anxiety Scale （SAS） scores， and Visual Analogue Scale （VAS） pain scores. This case report indicates that incorporating auricular acupressure into the ERAS pathway can effectively alleviate perioperative stress responses in lumbar UBE surgery， improve pain， anxiety， and sleep conditions， accelerate postoperative recovery， and enhance patients' self-management ability.
