摘要

报告1例79岁反流性食管炎伴重度胃肠胀气、顽固性便秘患者的中西医结合护理过程。患者以反酸、烧心反复发作3个月入院，伴嗳气、腹胀、食欲减退、大便干结半月一行、排气减少及睡眠欠佳。西医予抑酸治疗，中医辨证为吐酸病（脾胃湿热、气机郁滞、腑气不通证）。在常规护理基础上，采用神阙穴穴位贴敷联合隔药灸，配合情志护理、腹部按摩、体位护理与饮食护理。干预7 d后患者症状显著改善，体现了中西医结合护理在高龄复杂脾胃病患者中的应用优势，可为临床护理提供参考。

Abstract

This paper reports the integrated traditional Chinese and Western medicine nursing care delivered to a 79-year-old patient with reflux esophagitis complicated by severe gastrointestinal flatulence and refractory constipation. The patient was admitted with recurrent acid regurgitation and heartburn persisting for three months， accompanied by belching， abdominal distension， poor appetite， dry and infrequent bowel movements occurring once every two weeks， reduced flatus， and impaired sleep. Western medical treatment consisted of acid suppression， whereas traditional Chinese medicine （TCM） syndrome differentiation identified the condition as "acid regurgitation disease" （pattern of dampness-heat in the spleen and stomach， qi stagnation， and obstruction of fu-organ qi）. In addition to routine nursing care， the patient received Shenque （CV8） acupoint application combined with herb-separated moxibustion， together with emotional care， abdominal massage， postural nursing， and dietary guidance. After 7 days of intervention， the patient's symptoms were markedly alleviated. This case demonstrates the advantages of integrated Chinese and Western medicine nursing in the management of complex digestive disorders in elderly patients and may provide a reference for clinical nursing practice.
